Where should the media reside? I have an 'alarm.wav' in ..\My Games\CMUD but when I do a #play alarm.wav - I get nothing. =[

My Games\CMUD\Mudname\Sounds\file.wav

At least that's where automatically downloaded sounds go.

Alternatively the following syntax works as well:

#PLAY "C:\full\path name\here"

The default location is your session folder\Sounds, but you could also specify an absolute path.
